Medication training

Medication training is important for all your staff. I can offer basic training on the most commonly used medications in your home. I like to do this as a case study and use a real resident from your home. I make it interactive and fun and a certificate can be given at the end. 

Administration of medication competency

Medication administration requires suitable training for staff, so they understand the importance of lots of steps, such as making sure the medication has actually been taken, stock count and correct documentation on the MAR. I can make sure your staff are competent by by training and observation. 

Setting up Homely remedies

CQC like to see the use of homely remedies in the homes as it means that residents have the same access to medication as they would in their own home. For example, if they have a headache and not signed up for any painkillers on the patients MAR chart, they would be able to take a paracetamol for their headache from the homely remedies. I can help set this up and champion a member of staff to continue it going forward.

MAR chart audits

I can come in and look at your MAR charts to see if there are any noticeable issues. I can then train staff on any issues that arise.

Policies and documents

I can review these to make sure they are up to CQC standard or help set them up if they have bot already been created . Examples of this would include a prn protocol, warfarin MAR chart, patch MAR charts and a policy for medication if a resident wanted to go out for the day. There are lots of simple fixes that cane be done such as lists on fridges about what to do if the temperature is out of range and communication sheets.

Proxy online ordering of medication

This is robust way of ordering medication for your residents electronically. I can liaise with your GP surgery to help get this set up for your home. I can then train your staff on how to use it and maintain the system going forward. This is a very simple, but complex process, but once it it set up saves a lot of time on your ordering and gives you a robust audit trail.

Transfer of care review

I can look at any new residents who come into the home and give then a robust medicines reconciliation service. I can make sure they are on the correct medication, doses and formulations and identify any medications that may be difficult to source. I can look at quantities and liaise with the GP to get an interim supply whilst the resident fits into the ordering cycle of the home. I can make sure staff are aware of any important doses or knowledge they may need to know about the new resident's medication. 

Preparing for CQC

If you have an upcoming CQC inspection, I can look at everything medication related to see if there is anything that may need improving. This can include MAR charts, stock room, CD register, any resident that is self-medicating, document, thickener storage, policies and prn medication.

Post CQC

If CQC have identified any areas of concern that are medication related I can help with advice or documents that may be required to support this improvement. 

Any other medication issues:

I have lots of examples of where I have helped with medication issues. 

  • Support a  resident who was self medicating to set up the correct paperwork and make sure her medications were locked in her room.
  • A resident diagnosed with Parkinson's on arrival into the home, no times on the mediation and only twice a day - no follow up with hospital - I sorted a consultant appointment and he didn't have Parkinson's!
  • Transfer of care - patient arrived with all liquid medications as had a PEG in hospital. No longer had a PEG and could take tablets.
  • Resident had a fall in the night. I reviewed the medication to find furosemide was given at night meaning she would need the toilet.
  • Setting up of hypo boxes for many diabetic residents
  •  Food first - menus designed with the chef to fortify food rather than nutritional supplements
  • Finding local pharmacies who have a contract to always stock end of life medication. 
  • Spacer devices - found a home using one for all residents, so training was given on the use of spacer and how to keep clean.
  • Education to residents on their medication and why they are taking it
  • Stock issues - too much stock, wastage, carrying over and booking in.
