About me......
I am a pharmacy professional registered with The General Pharmaceutical council. I have completed an 18 month care home medicines optimisation programme and I have over 25 years experience with medication reconciliation and medication issues and reviews within primary care and care homes. I have worked in clinical education training since 2017, working with Warwick Medical School and postgraduate pharmacy education. I have trained numerous care home staff in my 25 years experience. I decided to set up this service due to many care home teams being taken out of service by local integrated care boards. I found that medicines management is no longer being supported as it once was in care homes.

Medicines optimisation in care home pathway completed 2020
Qualified in pharmacy 1996
